| Live Within 1 ½ Miles (H) | Checked if the student lives within 1.5 miles from his/her enrollment building. |
| Parent/Guardian SSN | Enter the Parent or Guardian Social Security Number. |
| Reimbursement Amount($) | Enter the reimbursement amount. |
