Changed the Substantiated Event field from a checkbox to a dropdown with values Y and N.

Additionally, reinstated the Warning message: The Substantiated Event Indicator must be blank when the Restraint Type Code is equal to Physical, Timeout BIP, or Timeout No BIP.
