The fields listed below are under the Reasons, Interventions, and Outcomes panel.

Fields	Description
Reason	Select the appropriate reason for the student's office visit. State Code Equiv = CON is used to report Questions 8, 9a, and 9b on the Health Services Report. State Code Equiv = ILL, INJ, or OTH is used to report Questions 25a and 25b on the Health Services Report.
Intervention	Select the appropriate Intervention type. This data is used to report Question 23 on the Health Services Report.
Outcome	Select the appropriate outcome code. Only codes with state code equiv = HOME, ER, or OHP will be reported. This data is used to report Questions 25a, 25b, 26a, and 26b on the Health Services Report.
