Success Plan Summary Page

Use this page to manage success plans for multiple students. The page displays students who have existing success plans or who qualified for at risk reasons that have not yet been assigned to a plan.

Initially, the page's information includes each student's name, ID, building grade level, and other general information. To display a student's success plan information, click the

(Expand) icon to the left of the student's name.

Depending on the student's records, the following sections may display:

Unassigned Reasons

Displays at risk reasons that have been calculated for a student but have not been assigned to a plan.

Success Plans

Displays a student's existing plans.

To display these sections for all students, click the

(Expand) icon in the column header to the left of the Student Name column on the Search Results panel.

Fields

Search Results Panel

To display a student's success plan information, click the

 (Expand) icon to the left of the student's name.

To limit the display of students, click

(Additional options) on the Search Results panel to display the following options:

Show Only Students with Unassigned Reasons

Limits the display to students who have unassigned reasons. When this option is selected, it is replaced by the option Show All Students.

Hide Completed Plans

Limits the display to students who have incomplete plans. When this option is selected, it is replaced by the option Show Completed Plans.

Student Name

The student's full name in last name-first name-middle name/initial format. Clicking the Student Name link displays the Student Summary page.

Unassigned Reasons Section

To display the section, click the

 (Expand) to the left of the student's name. If the section does not display, the student does not have any unassigned reasons.

To add a plan for a student, select the section's Add icon, which displays the Add New Plan window.

Field

Description

Assign to Existing Plan

Select the plan to assign for the reason identified in the Reason field. The selections include each plan's start date and title.

If no selections are available, the student does not have an existing plan. In this case, you would need to add a plan by clicking the section's Add icon.

Use Template

Checked if the default settings from the template associated with the reason should be applied to the student's success plan.

The default settings can include core interventions, student goals and objectives, and parent goals and objectives that should be added when a success plan is created. You can modify these defaults as needed to personalize the plan to the student's specific needs.

The checkbox only displays if the reason has a template set up for it. For more information, refer to Reason Template Page.

Reason

Code and description identifying the reason the student qualified for, as determined by a calculation in either Attendance, Behavior, Progress Reports, Report Cards, or Testing.

Qualification Criteria

Code and description of the at risk criteria the student met to trigger the reason.

Entrance

Date that the student was identified for the reason. This date can correspond to the date of the calculation that generated the at risk reason or can be entered when a reason is assigned manually.

Exit

Date that the at risk reason no longer applied to the student. Exit dates may be generated by calculations or can be entered for reasons that were assigned manually.

Success Plans Section

If any plans have been marked sensitive for a student and you do not have security to view sensitive plans, the text Additional plans exist for this student that you do not have security for displays below the panel's title bar.

Field

Description

Plan Title

Title identifying the success plan. Click this link to display the Success Plan Detail page for viewing or updating plan information.

Plan Date

Date assigned to the student's success plan.

Complete

Date that the plan was completed. This may differ from the Exit date.

Reason

Code and description identifying the reason the student qualified for, as determined by a calculation in either Attendance, Behavior, Progress Reports, Report Cards, or Testing.

Qualification Criteria

Code and description of the at risk criteria that the student met.

Entrance

Date that the student was identified for the reason. This date can correspond to the date of the calculation that generated the at risk reason or can be entered when a reason is assigned manually.

Exit

Date that the at risk reason no longer applied to the student. Exit dates may be generated by calculations or can be entered for reasons that were assigned manually.
