Health Services Report

Menu path: select Regulatory from the main menu, select Reports submenu, select Reports, and then select Health Services Report. 

This section describes the Health Services Report processing data and instructions.

Prerequisites

  1. Verify the building has an Attendance View (menu path: select Administration from the main menu, select Attendance Setup submenu, select Setup, and then select Attendance View Setup) setup to include the Concussion Excused (CE) attendance code.
  2. Verify the Day Totals (menu path: select Attendance from the main menu, select All submenu, select Student, and then select Day Totals) calculation has been run.
  3. Verify State Code Equiv values are entered for the Medical validation tables.
  4. Verify medical data is entered and accurate for applicable students. Refer to the Student Procedures section of the documentation for descriptions of medical data entry.

Processing

Menu path: Select Regulatory from the main menu, select Reports submenu, select Reports, and then select Health Services Report.

Enter the following fields and click Run to generate the report.

Select the building for which you want to run the report.

Only 1 building may be run at a time.

Date RangeSelect the start and end date for the period to include in the report.
School Type

Select the radio button for the type of school:

  • Public
  • Private or Non-Profit

Where Does the Data Come From?

The eSchoolPlus software will only generate data for certain Questions on the Health Services Report. The section below describes Questions 1-35 on the report. Questions not reported in the software are noted in the table below.

It will then count the number of students with Gender = 'F' and sort students by grade = 'PK-12'

Calculated field

The download will count the number of students who have a State Code Equiv value = 'CON' selected for the Reason field on the Office Visits page.

It will then count the number of students with Gender = 'M' and sort students by grade = 'PK-12'

The download will count the number of students diagnosed with concussions who required accommodations.

The download will count the number of students with applicable diagnosis codes. Refer to the Diagnosis/Disorder Codes (Question 15) section for a list of applicable state code equivalents.

Starting from 07/01/2020 to 02/012021, the CT Health Service Query will retrieve correct number of diagnosis for students associated with a specific building.

The download will count the number of students with applicable allergy codes. Refer to the Allergy Codes (Question 17) section for a list of applicable state code equivalents.

The download will look for the designated emergency medications administered to students. Refer to the Medications section for a list of the state code equivalents. If the medication was administered to any student, the download will report 'Yes'.

The download will look for the designated special procedures performed on students. Refer to the Special Procedures (Question 23) section for a list of the state code equivalents. If the procedure was performed on any student, the download will report 'Yes'.

Menu path: select Medical from the main menu, select All submenu, select Student, and then select Office Visits 

The download will calculate the percentage of students who had office visits lasting 30 minutes or less. To calculate the office visit time, the software will use the Time In and Time Out fields on the Office Visits page.

Menu path: select Medical from the main menu, select All submenu, select Student, and then select Office Visits 

The download will count the number of students who meet the following conditions:

  • Reason field: State Code Equiv value = 'ILL', 'INJ', or 'OTH'
  • Outcome field: State Code Equiv value = 'HOME', 'ER', or 'OHP'

It will then calculate the percentage of students who meet these conditions.

The download will count the number of screenings completed for the following exams:

  • Body Mass Index (BMI)
  • Pediculosis
  • Nutrition
  • Mental Health Consultation Services

  • Vision
