Description

LRE Code

This field will store the student's current Least Restrictive Environment (LRE) Part A code and description.

This field is program tracked. Click on the hyperlink to update the start date or end date, start reason or end reason, and select the LRE Code value.

Serving District

If the student is being served for his/her Special Education instructional needs in a different district, click the hyperlink to assign it.

This field is program tracked. Click on the hyperlink to update the start date or end date and select the Serving District value.

Location ID

If the student is being served for his/her Special Education instructional needs in a building different from his/her Entry/Withdrawal building, click the hyperlink to assign it. If the student is receiving services outside of your district, you must also enter a Location ID in the other district.

Initial Referral Date

Enter the date that the student was initially referred to the Special Education program. Once entered, this date should not be changed except for data entry errors.

The Student Special Ed record requires a Referral Date if the student also has an Initial Eligibility Date reported.

Initial Eligibility Date

Enter the date that the student was initially eligible to the Special Education program. Once entered, this date should not be changed except for data entry errors.

Last IEP Review Date

Enter the last date of the student's last IEP Review.

Last Evaluation

Enter the last date that the student was evaluated for placement in a Special Education program.

Initial WA Service Date

This is no longer used for CEDARS Student Special Education Programs (K) file, beginning in the 2014-2015 school year.

Non-Public Agency

If the student is being served through a non-public agency the school district has contracted, click the hyperlink to assign it.

PreK Social Skills Entry

If the student demonstrates age-appropriate functioning, across a variety of settings and situations in social-emotional skills (including social-relationships) at entry into the special education preschool program, click the hyperlink to assign it.

PreK Social Skills Progress

If the student has shown any new skills or behaviors related to social-emotional skills since entry data was taken, click the hyperlink to assign it.

PreK Social Skills Exit

If the student demonstrates age-appropriate functioning, across a variety of settings and situations in social-emotional skills (including social-relationships) at exit of the PK program, click the hyperlink to assign it.

PreK Uses Skills Entry

If the student is enrolled in Grade Level PK for more than one school year, continue to report initial data indicated by assessments and based on observations from individuals in close contact in each consecutive school year until the student exits PK, click the hyperlink to assign it.

PreK Uses Skills Progress

If the student has shown any new skills or behaviors related to acquiring and using knowledge and skills since entry data was taken, click the hyperlink to assign it.

PreK Uses Skills Exit

If the student has shown behaviors and skills as indicated by assessments and based on observations from individuals in close contact with the child, click the hyperlink to assign it.

PreK Behaviors Entry

If the student shows behaviors and skills as appropriate for their age across a variety of settings and situations, click the hyperlink to assign it.

PreK Behaviors Progress

If the student has shown any new skills or behaviors related to appropriate behavior and skills since entry data was taken, click the hyperlink to assign it.

PreK Behaviors Exit

If the student has shown behaviors and skills as appropriate for his or her age across a variety of settings and situations, click the hyperlink to assign it.

Annual IEP Meeting Date

If the student has not yet had an annual IEP team meeting, then enter the date of the most recent IEP (this is not the IEP start date and this does not include amended IEPs), click the hyperlink to assign it.
