FitnessGram Download and FitnessGram Extract

Updated the FitnessGram download and extract to match the 2022-2023 FitnessGram requirements:

• The Gender field has been renamed as SexAssignedAtBirth.

• The Race Ethnicity field has been replaced by breaking out into individual races/ethnicities: Hispanic or Latino Ethnicity, American Indian or Alaska Native, Asian, Black or African American, Native Hawaiian or Other Pacific Islander, and White.

• Individual Races and Ethnicities are determined from the Personal page.
