Create Student Success Plans

Use this procedure to step through adding a success plan for a student. The success plan includes the reasons, interventions, student goals, and parent goals related to it.

When you add a plan, you'll first assign a reason for the plan. If you have access to add a plan, you can add a reason for the student or you can select from an existing manual success plan reason. The list of available reasons for a student will not include reasons that were calculated for the student by an at risk calculation.

Create a student success plan

  1. From Interventions, select SSP Plans to open the Student Success Plans page.
  2. Click
    to select the student.
  3. On the Success Plan Entry Student Search window, enter text in field at the top of a column to search for a student. Click Search.
  4. Select the student, and then click OK.
  5. In Plan Manager field, verify that your information is displayed. If the login ID you use is assigned to multiple staff members and the teacher information displayed is not your information, click the (Not You?) link and select your staff record on the Teacher Chooser window. Click Ok to keep your selection and close the window.
  6. Click Reasons to add reasons. On the Success Plan Reasons pop-up, select an unassigned reason or add a new reason. Then click Save. You must add a reason before you can define the success plan that will address it.
  7. Select the Plan Type and enter the Plan Date and Plan Title.  As needed, enter other plan information.
    • If the plan should only be viewed by eSchoolPlus users with security to view sensitive plans, select the Sensitive checkbox.
    • In Qualification, enter a description of why the plan was created for the student.
  8. Click Save.
  9. Add student goals, parent goals, and interventions as needed.

View student success plan

You can view plans you manage and, if enabled, plans for students you teach in a class, homeroom, or activity. As needed, update the plans you manage to add goals and interventions. Plan Date and Plan Type cannot be changed.

  1. From Interventions, select SSP Plans to open the Student Success Plans page.
  2. To display plans managed by other teachers for any students you have in your classes, homeroom, or activities, select the Show Plans for Other Teachers checkbox. If you change this selection, click Refresh. The availability of this field depends on the success plan configuration.
  3. If the login ID you use is assigned to multiple staff members, a Teacher field displays with the teacher name and a (Not You?) link next to the teacher name. If the teacher information displayed is not your information, click the (Not You?) link and select your staff record on the Teacher Chooser pop-up. Click Ok to keep your selection and close the pop-up.
  4. To update a plan, click the plan link.
  5. On the SSP Plan Detail page, make changes to the plan information and add student goals, parent goals, and interventions as needed.

Add student goals for plan

  1. From the SSP Plan Detail page, click Student Goals.
  2. On the SSP Goals - Detail page, enter the details for the goal.
    • Select the goal and enter text describing the details.
    • In the Baseline field, enter text describing the student's situation at the time the goal was implemented.
  3. Click Save.
  4. To navigate back to the SSP Plan Detail page, click the plan link.
    After you have added a student goal, you can view the details for the goal by selecting Interventions > SSP Goals, and then navigating to the plan.

Add parent goals for plan

  1. From the SSP Plan Detail page, click Parent Goals.
  2. On the SSP Parent Goals – Detail page, enter the goal information.
  3. Click Save.
  4. To navigate back to the SSP Plan Detail page, click the plan link.

Add interventions for plan

Add interventions that you are responsible for administering. If another teacher will evaluate the student's progress, then the intervention must be added in eSchoolPlus. Interventions added in Teacher Access are assigned to the teacher creating the intervention.

  1. From the SSP Plan Detail page, click Interventions.
  2. On the SSP Interventions page, enter intervention information.
    • In the Role field, select the role appropriate for your participation in the intervention.
    • In the Intervention field, select the type of intervention.
    • If the intervention information should only display in eSchoolPlus for users with security to view sensitive plans, select the Sensitive checkbox.
    • In the Frequency field, select how often progress will be reported.
      As Needed - Progress needs to be reported as needed for the student.
      Each Day - Progress needs to be reported daily for the student.
      Selected Dates - Progress needs to be reported on the selected dates for the student. If you select this option, then use the Selected Dates field to select the dates.
      Weekly - Progress needs to be reported weekly for the student. If you select this option, then select the day in the Weekday field.
  3. Click Save.  
  4. To navigate back to the SSP Plan Detail page, click the plan link.

Delete Student Success Plans

If a plan has interventions or student goals associated with it, you cannot delete it.

  1. From Interventions, select SSP Plans to open the Student Success Plans page.
  2. If the login ID you use is assigned to multiple staff members, a Teacher field displays with the teacher name and a (Not You?) link next to the teacher name. If the teacher information displayed is not your information, click the (Not You?) link and select your staff record on the Teacher Chooser pop-up. Click Ok to keep your selection and close the pop-up.
  3. Select the Delete checkbox for the plan.
  4. Click Delete.
