Success Plan Coordinator Page

Use this page to define where classroom issues are referred. Records can be set up for different types of issues to determine whether they will be assigned to students' counselors or success plan coordinators.

When a class issue is referred to Student Success Plan, a counselor or coordinator can escalate the issue into a success plan or return it to the teacher. For counselors, assignments are based on the counselors identified in students' Registration Information pages. For coordinators, staff members can be assigned by name and then by certain criteria:

  • Issues can be grouped by referral tags. When a referral is made, the system assigns the proper coordinator based on the referral tag that applies.
  • Additional filter criteria can be set up, based on the personal, demographic, and academic fields in student records.

View Success Plan Coordinators

  1. Select Administration > Interventions Setup > Success Plan > Success Plan Coordinator.
  2. Within the Coordinators panel, you can view Success Plan Coordinator records in individual Definition sections.
  3. Refer to the Definition section for the record you want to view.

Add Success Plan Coordinators

  1. Select Administration > Interventions Setup > Success Plan > Success Plan Coordinator.
  2. Click Add. A new Definition section displays within the Coordinators panel.
  3. In the new Definition section, complete the fields as needed.
    • You can assign students to a Success Plan Coordinator using the Filter section.
    • Filters are not available for counselors.
  4. Click Save.

Change Success Plan Coordinators

  1. Select Administration > Interventions Setup > Success Plan > Success Plan Coordinator.
  2. In the Definition section for the Success Plan Coordinator, change the values as needed.
  3. Click Save.

Assign students to a Success Plan Coordinator

  1. Select Administration > Interventions Setup > Success Plan > Success Plan Coordinator.
  2. In the Definition section for the Success Plan Coordinator you want to assign students to, complete the fields in the Filter Panel as needed. For more information, refer to the Filter Panel field description in Success Plan Coordinator Page.
    Filters are not available for counselors.
  3. Click Save.

Delete Success Plan Coordinators

  1. Select Administration > Interventions Setup > Success Plan > Success Plan Coordinator.
  2. In the header of the Definition section of the Success Plan Coordinator you want to delete, click 
     (Delete).
  3. Click Save.

Building

Building in which the student is enrolled.

Referral Tag

Select the referral tag for the types of issues that should be referred to the staff member. Referral tags are assigned to issues on the TAC Setup Issues Page.

If more than one referral tag should be assigned to a staff member, you may enter as many records per individual as needed, each with a different tag.

To refer all issues to a staff member, do not select a referral tag.

Referral tags group issues that should be referred to the same staff member for schools that have multiple staff positions who evaluate students for success plans. For example, issues may be referred to an academic advisor, school psychologist, or counselor depending on the nature of the issue. If tardiness and missing homework issues are both considered academic issues that should be referred to the academic advisor, then these issues could be tagged with the same referral tag.

Referral Order

Number indicating the order in which referrals will be assigned. For each building, number the records beginning with 1. You cannot use the same number twice for the same building. When you click Save, the system sorts the records in the Success Plan Coordinator page by building code, then by referral order.

When multiple records are set up for a building, you should order them from the most specific to least specific. For example, if you have two records that have an Academic referral tag, one record could use a filter to identify ESL students, while the other might have a more general filter or no filter at all. To ensure that the coordinator responsible for ESL receives all related referrals, you would assign 1 as the referral order for the first record and 2 for the more general record.

Refers To

Indicates whether referrals are sent to a counselor or success plan coordinator.

Select:

Counselor - to assign referrals to students' designated counselors.
SSP Coordinator - to assign referrals for the current building to the coordinator identified. If a filter is used, plans will be assigned to coordinators accordingly, for example, all Academic issues or all students whose last names start with the initials A to K.
