The Counselor name in the transcript header shows Last Name, First Name + Middle Name from the appropriate staff member's Staff District Information page (Scheduling > Courses > Resources > Staff).

The student's birth location prints as long as the student Personal page has Birth Country and Birth City.

Leave Date prints if the student has a status of Graduated or Inactive.

Line 1 of the Student Address prints Complex from the Addresses page's Mailing Address section, if this data exists. The address does not include the student's phone number.

The Gender field will not be present in the OR Student Transcript file as per the DOE requirements.
