OR Medical Card Report Summary

An "Overall" Vaccination page is followed by pages for individual vaccinations. This report shows a breakdown of how many students have each Immunization Status, by grade level, for vaccinations set up with a State Code Equivalent in the Vaccination validation table. The report shows results based on the students selected by your report criteria.

The Grade KG to Grade 6 results for Polio is highlighted so you can compare them to the Section F Immunization Primary Review Summary results illustrated in the next section.
