OR Medical Card

Menu path: Select Regulatory from the main menu, select Reports submenu, select Reports, and then select OR Medical Card.

Use this page to create the Immunization Primary Review Summary (Sections A, B, C, D, E, F, and G); each building is required to submit to the OR Department of Health. You can also print summary and detail reports based on your selections for the report prompts. Refer to the prompt descriptions that follow for information on how to run the report.

The report only prints shots defined with a State Code Equivalent value in the Medical Vaccinations validation table (menu path: Select Administration from the main menu, select General Setup submenu, select District, and then select Validation Tables. You must select Oregon State Reporting for Applications and Medical Vaccinations for Validation Table). Grade levels are processed only if they have a State Code Equivalent value set up in the Registration Grades validation table (menu path: Select Administration from the main menu, select General Setup submenu, select District, and then select Validation Tables. You must select Oregon State Reporting for Applications and Registration Grades for Validation Table).

Building

Select the building to process.

New Page for each student

Checked if the Medical Card Detail report should provide a page break for each student processed. If this box is unchecked, the Detail report prints students as it has room on each page.

The Detail report prints only if the Include Sections B, C, and D box is checked.

Include Section A

Checked to print Immunization Primary Review Summary Section A, along with a separate summary report showing the number of students with each immunization status for each processed immunization series, organized by grade level.

Include Sections B, C, and D

Checked to print Immunization Primary Review Summary Sections B, C, and D, along with a separate detail report showing short records for each student included in the report run.

Include Sections E

Checked to print Immunization Primary Review Summary Section E.

Include Sections F, G, and HChecked to print Immunization Primary Review Summary Sections F, G, and D, along with a separate detail report showing short records for each student included in the report run.

Public/Charter School, Private School, Preschool/Day Care, and Head Start Programs

Select the grade level codes used to indicate Public/Charter School, Private School, Preschool, Day Care, and/or Head Start program if you are processing a building that includes any of these grades. Students in the selected grades are counted in Section E.

Start Date

Start date for the date range the report uses to select students. The report includes students enrolled in the selected building (in Entry/Withdrawal records) within the date range formed by the Start and End Dates.

End Date

End date for the date range the report uses to select students. The report includes students enrolled in the selected building (in Entry/Withdrawal records) within the date range formed by the Start and End Dates.

Log Statistics

Checked if you want to print the prompts and selection criteria used to run the report. This information prints as the first page of each report output.

Filter Criteria

Enter criteria to limit the students processed.

Section F of the Immunization Primary Review Summary counts totals for students in Grades Kindergarten through 6th. Similarly, Section G counts totals for students in Grades 7 through 12. You can limit processing using Entry/Withdrawal Grade if you wish.

Additional Information

The OR Medical Card Report will process the student name from the Personal page. If the student has Legal First name on the Personal page, the student name will be retrieved from the Personal page. If the student’s Legal First name is blank on the Personal page, the student name will be retrieved from the Registration page.
