School Year

The current school year is defaulted.

Student ID/Name

Student ID number of student transferring. Name field will automatically be populated after the Student ID is entered, or click the dropdown to search by name.

Withdrawal Date

Date the student withdrew or will withdraw. Will be automatically populated if the latest entry/withdrawal record for the student has a withdrawal date.

Contact Name and Contact Phone

Name and Phone Number of the contact at the sending school that can provide additional student information.

View Type

The view type to be used to get the days present and days absent. 

Absence Codes

The absence codes to be used to get the days present and days absent. Required if the View Type field has a value.

Mark Type

The type of mark to be used to print the grades on the form. 

Health Considerations?

Select Yes, No, or N/A. Selecting N/A will leave the Blood Lead Screening & Physical Exam fields blank on the SR7 Report.

Enrolled in Gifted/Talented Program

Checked if the student is enrolled in the Gifted/Talented program.

Enrolled in Other Supplemental Programs?

Checked if the student is enrolled in a Supplemental program not included in the other listed programs.

Immunization Compliant?

This field will be active if information for this is not found on a user-defined screen determined by the MD Defined Fields entry for 'IMMUNE'. If a value exists, the field will be populated and deactivated. Otherwise, checked to indicate that the student is up to date on immunizations.

Service Learning Hours Completed

This field will be active if information for this is not found on a user-defined screen determined by the MD Defined Fields entry for 'SERVICEHRS. If a value exists, the field will be populated and deactivated. Otherwise, enter the number of service learning hours the student has completed.

Not Attending School Due to Disciplinary Action?

Checked if the student is not attending school as of the withdrawal date due to disciplinary action.
