ESP-61165VA SBAR Fields and Validation Tables

Added “VA SBAR Fields” district-defined page as per the VA 21-22 SBAR Submission Discipline Action requirement.
Added five fields (Behavior Offense Code, Behavior Intervention Code, Instructional Support, Alt Placement Ed. Agency, and Alternate Placement School) to the VA SBAR Fields district-defined page.
Added validation table for VA Behavior Intervention Codes with valid values.
Added validation table for VA Instructional Support Codes with valid values.
