Enter the unique ID for the residential facility where the student should attend based on their residence.

Educated At Facility (e1634)

Select if the student received instruction at or in the general location of a residential facility.

Surrogate Parent (e1635)

Select if the student was assigned a surrogate parent.

Surrogate Parent Other Stu Cnt (e1636)

If applicable, enter how many other students have the same surrogate parent as the student.

RF Instructional Minutes (e1637)

Enter the number of minutes the student receives instruction at the residential facility.

Non-RF Instructional Minutes (e1638)

Enter the number of instructional minutes in a school day for the residential facility campus where the student is enrolled.

Prior Instructional Setting (e1639)

Choose the location where the student received instruction before enrollment at the residential facility.
