Reports

This section describes the reports that are part of Texas state reporting.

There are several reports provided for state reporting. Unless noted otherwise, these reports are available on the Regulatory - Reports menu.

Description

Attendance Reports

Attendance Verification

Used to account for the membership and attendance of all students on campus. The report lists students who are enrolled in a course or homeroom attendance period and any attendance those students have during that time period.  It includes attendance for courses taught outside the student's enrollment building. Students who are active for the date, but are not scheduled into a course or homeroom for the selected periods will also be included in the report.


Enrollment Verification

Used to account for the membership of all students on campus. The report lists students who are enrolled in a course or homeroom on a particular date and scheduling period.  Students who are active for the date, but are not scheduled into a course or homeroom for the selected periods will also be included in the report. Based on your prompt selections, you can include courses taught outside the student's enrollment building or periods from alternate cycle day timetables.


District/Campus Summary

Used to print membership and attendance data by grade, contact hours by vocational code, and instructional setting information for the District or for a campus.


OFSDP District/Campus Summary

Used to print membership and attendance data by grade, contact hours by vocational code, and instructional setting information for students enrolled in the OFSDP program.


Student Detail

Used to print student demographic information, membership and attendance information by attendance interval, and program contact hours information. 

Medical Reports

Annual Report of Immunization Status

Used to print immunization compliance and exemptions with district totals by grade. 


Antes Screening

Used to print the University of Texas Pan-American Border Health Office (BHO) Type II Diabetes Risk Assessment Consolidated Data Form.


Hearing Screening

Used to print the Texas Department of State Health Services Hearing Screening report.


Spinal

Used to print the Texas Department of State Health Services Spinal Screening report.


Vision Screening

Used to print the Texas Department of State Health Services Vision Screening report.

Verification Reports

90 Staff

Used to verify, for each staff member with a State Staff ID, the courses and responsibilities associated with the staff. The report will also show the generated 090 records for the same staff member.  If the number of courses/responsibilities and the number of 090 records do not match, a warning message will display on the report. You can print this as a PDF or Excel file.


100/101 Demographic

Used to verify student data as it has been calculated into the 100/101 records. Student information from the Registration, Personal, and Entry/Withdrawal pages and Program enrollment information will be printed along with the data from the 100 and 101 records for each student. 


110 Enrollment

Used to verify student data as it has been calculated into the 110 record. Student information from the Registration, Personal, and Entry/Withdrawal pages and Program enrollment information will be printed along with the data from the 110 record for each student. 


203 Leaver

Used to verify student data as it has been calculated into the 203 record. Student information from Entry/Withdrawal is printed along with the data from the 203 record for each student. You can print this as a PDF or Excel file.


300 Course

Used to verify course-section data as it has been calculated into the 300 record. Master Schedule data is printed along with the data from the 300 record for each course-section. To identify non-high school courses, the report references the Pass/Fail Setup's Non HS Mark Types field.


305 Teacher Assignment

The report shows staff course schedules compared to the 305 records created for those staff members by building. You can limit the report to show staff lacking any 305 records or staff with missing 305 records. You can also select to run for all staff, primary staff, or secondary staff. To identify non-high school courses, the report references the Pass/Fail Setup's Non HS Mark Types field.


400 Attendance

Used to verify Attendance Day Totals and PEIMS ADA Attendance information for each student for the selected reporting periods. 


405 Attendance

Used to verify the Attendance Day Totals and PEIMS Special Education information for each student for the selected reporting periods. 


410 Attendance

Used to verify the Attendance Day Totals and PEIMS Career and Technology information for each student for the selected reporting periods.


415 Course Completion

Used to verify student Mark Reporting course data and some Master Schedule district-defined information compared to the 415 records created for those student courses. You can limit the report to show students lacking any 415 records or students with missing 415 records. You can also select to print student Entry/Withdrawal information. To identify non-high school courses, the report references the Pass/Fail Setup's Non HS Mark Types field.  The report shows scheduled and non-scheduled courses, including those without grades.


425 Discipline

Used to verify current discipline data along with the corresponding 425 record data for each student that meets the entered selection criteria. Users can select all students or only those with missing 425 records.


461 Title I Part A

Used to verify the student's Title I Part A program records with the corresponding 461 record data for students. Users can select to run for all students, only students missing 461, or only students with discrepancies in data.

Testing Reports

End of Course

Use to create the data file to provide to Pearson for the End of Course (EOC) Assessments. The data file will include students who are scheduled for the state courses on the as of date.


STAAR/TAKS/TELPAS Pre-code

Used to create a data file or print a report with basic demographic information and program data for students who will be taking the TAKS test, STAAR 3-8, or the TELPAS assessment.


Student EOC Requirements HB5

Used to create a Microsoft Excel file with End of Course exam results for the students and tests you choose. The report lets you print any or all of the tests required based on House Bill 5, 83rd Texas legislature. The Excel file includes formatted and sortable versions of the report.  You can choose whether to include student notes from the Student EOC Requirements HB5 page.

Additional Reports

Student Courses Across Buildings

Used to create a report of students who have completed different parts of a high school state course in multiple buildings.

Menu path: Select Regulatory, Reports, Other, and then choose Student Courses Across Buildings.


Withdrawal Form

Used to print a detailed withdrawal form for students who leave a building in your district. You select a student, withdrawal date, and withdrawal code. You can select test information to print, and prompts are available to run calculations for Attendance Contact Hours and Report Card Course Attendance if needed.

Menu path: Select Regulatory, Reports, Other, and then choose Withdrawal Form.

Registration Reports

Calendar Day Report

Used to create a report of the calendar days in multiple buildings.

Menu path: Select Regulatory, Reports, Registration, and then choose Calendar Day.
