Database and Validate Immunizations

Created MED_SHOT_DET_OR table with associated AFTER INSERT and AFTER DELETE triggers, and included in the OR02_tables.sql script.

Updated the MED_SHOT_DET_OR script to calculate immunization totals and statuses from the Database by the State Reporting team.
