The information on this page is used in the Family Care Details report. This report generates a CSV file indicating whether selected students are insured, qualify for free or reduced lunch, and have parental consent for information-sharing with the Family Care Outreach program.
The Details report file is submitted to the New Jersey Family Care Outreach program. Students' eligibility is then assessed for low-cost health insurance.
Field | Description |

Parental Permission | Checked if the student's parent or legal guardian has given the district permission to share the child's information with NJ Family Care. The Family Care Details report will only include a student if this box is checked. |
Uninsured | Program-tracked indicator showing whether the student lacks health insurance. The Family Care Details report will only include a student if the program's Uninsured box is checked for the date range when the report is run. |
