Comment

Additional comments your district can use for your own records related to the student's Early Childhood program participation. This value is reported in the Fall, Spring, and End of Program Early Childhood collections.

Provider License Number

License number assigned to the location where the student receives early childhood services. This is either formatted with 2 letters followed by 9 digits, or it needs to begin with tribal.

Federal Poverty Level

Federal Poverty Level quintile which a child qualifies for a Great Start Readiness Program or GSRP/Head Start Blend program.

Eligibility Factor (7 fields)

Select the eligibility factors used to determine the student's eligibility for a Great Start Readiness Program. These factors are in addition to the income information provided in the Federal Poverty Level field.

  • If the Early Childhood Enrollment is either 01 or 02, the student needs at least one Eligibility Factor. The SDS Download defaults 08 - None if there are no Eligibility Factors selected for the EC Enrollment and date range.
  • If the Early Childhood Enrollment is not 01 or 02, then do not report any Eligibility Factors for the student.
  • If you report an Eligibility Factor of 04 for the student, then you also need to report the student in the Title III English Learner and Immigrant component.
  • If you choose 08-None for any of the Eligibility Factor fields, the SDS Download will report 08 in the Early Childhood Programs component, regardless of whether the student has any other Eligibility Factors for the program and date range.

Qualifying Factor (4 fields)

Select the qualifying factors considered when the student's eligibility was determined for a Great Start Readiness Program. A qualifying factor automatically prioritizes the child for enrollment.

  • If the Early Childhood Enrollment is either 01 or 02, the student needs at least one Qualifying Factor. The SDS Download defaults D - None if there are no Qualifying Factors selected for the EC Enrollment and date range.
  • If the Early Childhood Enrollment is not 01 or 02, then do not report any Qualifying Factors for the student.
  • If the student has a Qualifying Factor of A - Homeless and/or B - Foster Care, then the reported Federal Poverty Level must be 1 - 0% to 50% FPL.
  • If the student has a Qualifying Factor of A, then he or she also needs an Eligibility Factor of 07 - Environmental Risk.
  • If the student has a Qualifying Factor of C - Qualifying IEP, then he or she also needs an Eligibility Factor of 02 - Diagnosed disability or identified developmental delay.
  • If you choose D - None for any of the Qualifying Factor fields, the SDS Download will report D in the Early Childhood Programs component, regardless of whether the student has any other Qualifying Factors for the program and date range.

Working Parents

Checked if the child's parent(s) work and cannot care for the child during the program hours for the indicated Early Childhood Enrollment.

Clear this box to indicate the parents do not work during program hours and are available to provide child care.

Fiscal Entity Code 2

Second entity receiving state funding for providing the educational service to the reported child. This applies in the case of a blended program. Use the Educational Entity Master number for the entity receiving funding for providing the second part of the blended service.

You can only report this if the Early Childhood Enrollment program is 02 - GSRP/Head Start Blend).
